~ Patient Privacy Policy (HIPPA Compliant) ~

All information given during your visit with Dr. Michelle Wolford, ND is kept confidential and solely between the patient and doctor. The only time this commitment of doctor-patient confidentiality is broken is if the doctor feels her life or the life of the patient is threatened. With the signature of a minors guardian authorizing a consent to treat, the doctor-patient confidentiality protects the minor from anything information disclosed in the visit to the guardian unless there is sign of abuse or potential risk of harm to self or others.

 ~ Terms and Conditions ~

Each patient is required at the time of visit to pay for their visit via cash, credit card or check. American Express is not authorized at this location. If a check bounces, a $50 claim will be submitted against the payee. If the patient arrives late to a visit, the visit will continue to end at its scheduled time. There are no refunds on supplements unless the doctor feels the reason for product return is viable. Supplements are ordered at the end of the business day every Thursday. The patient is required to re-pay for supplements ordered and can request home delivery for an additional fee of $8.50, otherwise the supplements will be ready for pick up at the office the following Thursday.

~ Cancellation Policy ~

Cancellations must be made at least 24 hours prior to agreed appointment via phone at (760) 230.2270 or e-mail at DrMichelle@Lifeinmotionmed.com. If you do not comply you will be charged a Missed Appointment Fee of $50.00 for missed appointments or late cancellations with less than 24 hours.

~ Insurance ~

Life In Motion, Naturopathic Medical Clinic is not a provider for any insurance plan. If your plan has coverage for out-of-network naturopathic providers, Dr. Michelle can provide you with appropriate coding to submit your own insurance claim.
